​Insurance Requirements

Just a reminder to let you know pursuant to the Madera Vista Townhouses, Inc CC&R’s Article XII, declares a copy of your Homeowner’s Insurance with coverage period is due annually.  You have 3 ways to provide this to Jim Crowley, one of our  Board of Directors.  First you can provide the declaration page from your insurance company, second you can fill out the form at the link below and sign it then either email or mail to Jim Crowley at 366 S. Avenida de las Sabinas, Green Valley, AZ. 85614, or third you can send an email stating the coverage period of your policy at mvhoa104@mail.com. 

Under policy starting in 2019 if the HOA does not receive proof of insurance within 3 months of this notification you will be fined $20 for each month you continue to be delinquent.  This fine will be added to your annual HOA dues or will be levied against your home upon selling.
